    The Nursing Supervisor:

    • Conducts monthly Nurse meetings to perform in-services and promote educational training to clinicians based on Education Plan and identified regional skills needs
    • Performs weekly review of clinician’s productivity and report to Director, Clinical Operations and counsels team members regarding ongoing productivity expectations
    • Functions as the primary resource to associate questions and concerns. Provides guidance to clinicians regarding questions concerning proper patient care.
    • In collaboration with Rehabilitation Supervisor, leads all weekly case conferences to review admissions, discharges and re-certifications and other patient specific needs. Assures discharge planning is completed and required documentation is assigned and/or returned for upload to chart.
    • Collaborates with the scheduler daily to review staffing needs to properly serve patient census – collaborates proactively with other regional Supervisors to assure shared staffing needs are met by requesting and offering staffing assistance
    • Reviews on-call logs daily, assures that patient on-call needs have been addressed and files on-call logs in On-Call Binder
    • Assists in new hire orientation, provides written calendar with preceptor assignments through the first 30 days of employment to HR and ensures training is completed to agency expectations
    • Monitors overtime, absenteeism and associate ETO requests on a weekly basis – allows no more than one clinician from each discipline to be “off” at any time to assure patient coverage.
    • Coordinates 24 hour on-call coverage per region, creates on-call calendar for each month and presents calendar to scheduling at least 3 calendar days prior to the first day of each month and acts as a back-up to provide patient care if needed. Creates annual holiday on-call calendar with updates throughout the year as needed.
    • Tracks and reviews Tele-Health utilization and determine any patients that are candidates for the program
    • Manages and directs a team of clinicians including RN Case Manager’s, RN’s, LPN’s, Social Work and Home Health Aide’s to ensure a safe, effective, and appropriate home care service is provided
    • Meets with clinicians regularly on an individual basis to provide guidance and information related to specific issues. Documents clinician counseling when provided per progressive disciplinary policy in collaboration with HR and Director
    • Provides direction to teams to assure client needs are met and services are provided per the agency policies and procedures
    • Performs annual supervisory visits (“ride-alongs”) with clinicians to determine staff competency and respond to educational and developmental needs and documents visits with appropriate tool
    • Assists clinical staff in establishing priorities, setting goals and following up to ensure goals are met by conducting performance reviews in a timely manner. Presents orientation “90 day” review prior to the end of clinician’s initial 90 days of employment.
    • Collaborates with the Director, Clinical Operations on process improvement, quality initiatives and policy/procedure implementation
    • Educates clinicians on clinical services, policies, and procedures as needed
    • Facilitates problem-solving sessions to enable Case Managers and other staff to resolve client and/or reimbursement issues.
    • Serves as the clinical expert on home health guidelines, ACHC requirements, State Operations Manual and Medicare regulations to accurately respond to associate concerns and questions.
    • Ensures accuracy, completeness, and timeliness of clinical documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards – reviews staff task lists routinely to assure timely filing of documentation.
    • Monitors open and closed charts regularly, and participates in the quarterly clinical record review.
    • Assists in screening and interviewing of new Agency personnel and makes recommendations to Director, Clinical Operations and Human Resources.
    • Completes corrective actions and delivers necessary performance discussions in collaboration with Human Resources.
    • Provides direct patient care to adult and geriatric patients in the home to assist with staffing needs.
    • Maintains accurate and up to date schedules for field staff and actively plans for productivity
    • Provides on-call backup for Case Mangers, as needed.
    • Performs other duties as assigned by the Director of Clinical Operations.
    • Assures that patient supplies are ordered through appropriate vendor per patient’s payer, and that supplies are charged to the patient through EHR.

    Basic Qualifications

    • Graduate of an accredited school of professional nursing.
    • Minimum of 5 years' nursing experience in an acute care setting, preferably home health.
    • Minimum of 3 year' home health experience
    • Must possess current license from the Indiana State Board of Nursing.
    • Valid driver’s license and automobile insurance with reliable transportation to perform job tasks required
