§ Re-evaluates the need for continued care on an ongoing basis
§ Plans for discharge from the Agency with the patient and caregivers. Makes referrals for alternative level of care if necessary.

§ Assist Director of Operations and staffing coordinator with the hiring and orienting and disciplining of all clinical.
§ Graduate of an accredited school of nursing.
§ Registered Nurse with an active and unrestricted license in state of employment
§ Two years experience as a Registered Nurse in a clinical care setting preferred. Home Health preferred.
§ Current CPR card
§ Valid state driver’s license and reliable automobile.
§ Proof of current automobile insurance.
