Position: Home Nursing Staff Nurse

Location:  Travel between Freeport IL and Rockford IL

Type: PRN, weekly 1 hour visit per client

• Comprehensive Care: As a Home Nursing LPN/RN, you will be responsible for delivering skilled nursing services to our clients in the comfort of their own homes. Taking vital signs and filling medication planner according to treatment plans is the primary role.
• Personalized Approach: Create individualized care plans that cater to each client's unique needs, preferences, and circumstances. Build strong relationships and provide compassionate care that addresses both medical and emotional aspects.
• Patient Education: Empower clients and their families by providing education about their health conditions, medications, and self-care techniques to promote overall well-being.
• Effective Communication: Maintain open lines of communication with clients, their families, and other members of the healthcare team. Provide regular updates on clients' progress and any changes in their condition.
• Documentation: Keep accurate and detailed records of all nursing care provided, assessments made, and changes observed. Ensure continuity of care by following proper documentation procedures.

Qualifications:

• Active LPN or RN license in IL State
• Strong clinical background
• Strong clinical skills and a solid foundation in nursing best practices.
• Exceptional interpersonal and communication skills to interact effectively with clients, families, and healthcare professionals.
