Nursing Supervisor - Nursing Home jobs in San Antonio, TX

Responsibilities:
- Provide direct patient care, including administering medications, performing diagnostic tests, and monitoring patient vital signs.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Document patient information, including medical history, symptoms, and treatments, in electronic health record systems such as Cerner, Athenahealth, and eClinicalWorks.
- Educate patients and their families on health conditions, treatments, and preventive measures.
- Coordinate discharge planning and ensure smooth transitions of care for patients.
- Adhere to professional standards and guidelines in providing nursing care.
- Specialize in pediatric care or other specific areas of nursing as needed.
- Provide nursing care in various settings such as hospitals, nursing homes, hospice care facilities, etc.

Qualifications:
- Valid nursing license and registration.
- Knowledge of healthcare standards and regulations.
- Experience in health information management and electronic health record systems.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Compassionate and empathetic approach to patient care.
- Familiarity with HEDIS (Healthcare Effectiveness Data and Information Set) measures is a plus.
