Buckeye Home Health Care is seeking an Occupational Therapist who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients.

Qualifications:

• Ability to perform therapy evaluations, routine visits, and OASIS documentation
• Work with other disciplines to give the best care needed for each patient.
• Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
• When appropriate teach and counsel family members about care and processes for disease/injuries.
• Attend orientation, meetings, and CEUs
