    The RN is a member of the care team responsible for day-to-day coordination of care needs for our medically complex cases for our local office in Eugene, OR.     You will be responsible for thinking through the client journey and how we can continue to optimize the client onboarding experience from inquiry to assessment to client care and connection. Additionally, you and the team are responsible for the smooth coordination of operations to achieve quality agency standards and positive financial outcomes.

    Responsibilities:

    • Perform initial assessments for Clients with external Case Managers (Medicaid, VA, OPI, ODDS, etc.).
    • Perform Quarterly Assessments for all nursing clients.
    • Assist Care Managers with new Client Care Plans and Task Lists
    • Initial and follow-up records requests and processing for all Nursing Services Clients (Medication Administration, Delegation, Medically complex, and Private pay)
      • Includes H&P, Medication Lists, Doctors orders for delegated tasks.
    • Change in Conditions: Initial screening and escalation per RN protocols.
    • Audit electronic medication form submissions.
    • Evaluate and report quality of client-site services provided.
    • Provide initial client onsite binder; support and update as needed.
    • Identify, document and maintain records of client’s outside supports i.e. PCP, family members.
    • Develop home safety plans.
    • Identify client medication level of support.
    • Identify client physical, nutritional and hygiene supports to include transfer and mobility.
    • Identify data discrepancies and/or information deficiencies.
    • Escalate for resolution identified client and/or caregiver problems or concerns.

    Qualifications:

    • Current RN licensure in the State of Oregon in good standing
    • Reliable transportation and current Oregon Drivers License with auto insurance.
    • Effective knowledge of community based resources and services for the elderly.
    • Creative problem solving skills with ability to manage challenging situations.
    • Excellent time management and organizational abilities with strong team player attitude and team building skills.
    • Ability to handle multiple tasks simultaneously while determining priorities and managing a changing environment.
    • Ability to monitor and assess physical status of clients and plan, implement, and evaluate individual patient care programs.
    • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community and office workplace.
    • Ability to gather data and complex information while making autonomous decisions using best practice standards of nursing.
    • Ability to communicate medical information to health care professionals and the public over the telephone under normal and crisis circumstances.
    • Knowledge of the development of health care and client contracts preferred.
    • Ability to work independently and be self directed.
    • Must be conversant in the Oregon State Board of Nursing scope of practices and procedure.
    • Satisfy drug screening and criminal background screening.
