GHG Home Care is seeking a Scheduling Coordinator/Case Manager for our growing office.

A Scheduling Coordinator/Case Manager is the person who supervises and directs home health care aides that visit patients who need help in their homes with nursing, light tasks, and/or maintenance duties. The Coordinator typically helps to ensure that properly credentialed care personnel are working with patients.

Job Responsibilities:

• Responsible for scheduling of aides.
• Conducting client assessments and home visits to assess service needs and create and help execute service plans.
• Monitor hours of service delivered, quality of service delivery, client satisfaction and changes in client's functional status.
• Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction.
• Ability to travel at times to open new cases and visit current consumers.
• Ability to work overtime hours, take after hours calls and be on call for scheduling and Case Management

Requirements:

• Bilingual in both English and Spanish is a PLUS!
• Able to be on call for scheduling and Case Management
• Proven working experience in scheduling, case management or other related job field.
• Excellent knowledge of case management principles.
• Effective communication skills
• Excellent organizational and time management skills
• Problem solving skills and ability to multi-task
• Basic knowledge of Microsoft Word, Excel, and Google Sheets
• Ability to work in a team environment
• Excellent follow up skills as well as attention to detail
• Must have a Valid Driver’s License.
