Registered Nurse - Home Health

Overview:

We are seeking a compassionate and dedicated Registered Nurse to join the Generations Home Health Care team. As a Home Health Nurse, you will provide high-quality care to patients in the comfort of their own homes. This is a rewarding opportunity to make a positive impact on the lives of individuals in need of skilled nursing care.

Responsibilities:

- Conduct initial assessments and ongoing evaluations of patients' health status - familiarity with OASIS completion.

- Develop and implement individualized care plans based on patients' needs

- Provide skilled nursing care, including wound care, medication administration, and IV therapy

- Monitor vital signs and assess patients' response to treatment

- Collaborate with physicians and other healthcare professionals to coordinate patient care

- Educate patients and their families on disease management, medications, and self-care techniques

- Perform discharge planning to ensure a smooth transition from hospital to home

- Maintain accurate and up-to-date medical records

Requirements:

- Valid Registered Nurse (RN) license in the state of

- Minimum of 2 years of nursing experience in acute or critical care settings

- Strong knowledge of physiology, medical terminology, and medical records management

- Excellent clinical assessment skills and ability to recognize changes in patients' conditions

- Ability to work independently and prioritize tasks effectively

- Exceptional communication and interpersonal skills

- Compassionate and empathetic approach to patient care

Preferred Skills:

- Experience in home health or primary care settings

- Familiarity with Medicare guidelines for home health services

Job Type: Full-time

Pay: $45,000.00 - $68,000.00 per year
