JOB DESCRIPTION SUMMARY

Responsible for implementation of marketing plan and establishing and maintaining positive relationships with customers. Finding different referral sources and maintaining relationship with them. Accountable for new business growth and meeting reasonable goals. Creating and implementing new ideas. Bringing on new talents to the company. As a community Liaison you will be the contact for initial prospective clients. You’ll be assisting potential clients throughout the waiver application process. You’ll also be tasked to continue relationship with existing client base and maintain your caseload.

Detailed Description

• Someone with office and customer service experience
• Responsible to respond to inquiries from prospective patients.
• Deal with referrals, if needed physically travel to the patient’s or referrals location to get further information.
• Make or receive calls to/from physician offices, partners, social workers, other related agencies.
• Promote and educate regarding the services we offer.
• Understand the waiver and home and community based programs that we offer.
• Responsible to understand and follow appropriate procedures and stay within the regulations.
• Develops professional relationships with outside agencies to better assist incoming participants.
• Adhere to policies, procedures, and compliance requirements of Golden Home Care.
• Help enroll prospective clients to the waiver program or the Medicaid Program.
• Maintain a professional relationship with consumers and employees and different sources.
• Coordinate and communicate between office staff.
• Create and maintain relationship with other community organizations and/or professionals.
• Other tasks asked as required.

Bonus - Performance Based

depending on number retentions and new enrollments up 25% to 30% of the salary at the end of fiscal year

Job Type: Full-time

Pay: $21.00 - $24.00 per hour

Expected hours: 40 per week
