Outpatient Care Coordinator supervises the day-to-day activities of the patient relations team of a healthcare organization to deliver patient liaison services and patient-centric care. Coaches staff on best practices for responding to patient and family questions, concerns, and issues. Being a Outpatient Care Coordinator facilitates collaboration with multi-disciplinary teams to make recommendations for improvements to the patient experience. Ensures that escalated problems are addressed and resolved. Additionally, Outpatient Care Coordinator provides and maintains up-to-date informational materials and resources for patients and families. Follows all applicable regulations regarding patient information privacy policies. May require an associate degree. Typically reports to a manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members.     Job Summary:

    Performs procedures which require specialized training for general nursing care and related services for assigned patients, under the direction of a Registered Nurse (RN) or Medical Doctor (MD), in accordance with established policy. Participates in the planning for and provision of patient care. Uses knowledge of asepsis in performance of treatments, such as sterile dressings, irritations and cauterizations. Provides functional direction to ancillary personnel. Demonstrates a high level of knowledge/expertise, and is a leader/resource to the healthcare team.

    Job Requirements:

    Education and Work Experience:

    • High School Education/GED or equivalent: Preferred
    • Bachelor's Degree or equivalent combination of education/related experience: Preferred

    Licenses/Certifications:

    • Licensed Vocational Nurse (LVN) licensure in the state of practice: Required
    • Basic Life Support (BLS OR HS-BLS OR RQIBLS) Health Care Provider certification: Required
    • Arrhythmia certification: Preferred
    • Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) or Registered Nurse (RN) or Nurse Practitioner (NP): Required

    Essential Functions:

    • Provides care for assigned patients, under the supervision of an Registered Nurse (RN) according to established standard and nursing process. Works on problems of diverse scope and substantial complexity. Mentors and trains staff on technical skills.
    • Performs admission assessments, and ongoing re-assessments based on departmental specific policy and procedures. Assists in the development of a prioritized interdisciplinary plan of care on each patient.
    • Implements and coordinates plan of care utilizing appropriate organizational skills and reporting mechanisms. Implements individual teaching plans, including Patient Teaching Protocols and discharge Plans, based on mutual goal setting and with emphasis on patient decision making.
    • Performs assigned clinical procedures (i.e. phlebotomy, injections, drug screens, etc.) Administers medications.
    • Orders/restocks supplies as assigned, according to established par levels. Maintains neatness/cleanliness in assigned areas.
    • Performs other job-related duties as assigned.
