Outpatient Care Coordinator supervises the day-to-day activities of the patient relations team of a healthcare organization to deliver patient liaison services and patient-centric care. Coaches staff on best practices for responding to patient and family questions, concerns, and issues. Being a Outpatient Care Coordinator facilitates collaboration with multi-disciplinary teams to make recommendations for improvements to the patient experience. Ensures that escalated problems are addressed and resolved. Additionally, Outpatient Care Coordinator provides and maintains up-to-date informational materials and resources for patients and families. Follows all applicable regulations regarding patient information privacy policies. May require an associate degree. Typically reports to a manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members.     Job purpose

    The Intensive Outpatient Program (IOP) Coordinator is responsible for the administrative coordination of the Intensive Outpatient Program including communication among prospective and enrolled patients, IOP clinical staff, and FHCHC staff in general. They will also serve as a community liaison for the program by increasing awareness to the availability our programs and helping to facilitate enrollment.

    Duties and responsibilities

     

     

    Reporting to the Director of Behavioral Health Operations, the IOP Coordinator will:

     

    • Respond to all incoming inquiries for IOP program, including new patient inquiries submitted online, via telephone calls, and from FHCHC providers within 2 business days
    • Complete primary screen for prospective patients, collect insurance information, assign the patient an account number and schedule an appointment in accordance with clinician and/or program guidelines
    • Set up patient charts in EHR for new patients which includes following up with prospective patients to complete new patient documentation; update patient demographics as needed. Manage the IOP patient waiting list on a routine basis
    • Cancel/reschedule appointments as needed; notify appropriate clinic personnel. This also includes contacting patients who do not show up for the program.
    • Maintain documentation in EHR of all correspondence with patient
    • Assisting with interpreting, understanding and reviewing insurance benefits with prospective patients or their designee
    • Obtain initial and renewal authorizations for IOP services for every patient enrolled in program in a timely manner so as to not interrupt care.
    • Provide referral options if patients are not appropriate for IOP services at FHCHC based on clinician guidance
    • May assist in facilitating group therapy sessions and provide support in sessions by overseeing patient activities and interactions
    • Obtain and communicate messages in an accurate and timely manner
    • Coordinate with FHCHC BH Navigators regarding IOP patients who need an outpatient therapist following discharge
    • Represent FHCHC’s IOP Program to members of our community through targeted outreach with the goal of increasing enrollment
    • Perform other necessary duties required by FHCHC to provide high quality care.

    Qualifications

    • Bilingual with oral and written fluency in English and Spanish is required.
    • Bachelor’s degree and at least one year of experience in a in a health related field required. Additional relevant experience may replace the need for the degree requirement.
    • Ability to organize and prioritize tasks, time, and responsibilities effectively, as well as to work independently and with minimal supervision
    •  Superior interpersonal skills, ability to build relationships, and interact collaboratively with other departments, public and patients.
    • Experience in ambulatory or outpatient integrated care setting is highly desirable; familiarity with electronic health record system is essential, experience with Epic preferred.

    Physical Demands: Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer, pulling medical records, Mental Demands: Ability to cope with continual changing priorities under potentially stressful conditions. Manual Dexterity Required:  Ability to use a keyboard, telephone.
