Outpatient Care Coordinator supervises the day-to-day activities of the patient relations team of a healthcare organization to deliver patient liaison services and patient-centric care. Coaches staff on best practices for responding to patient and family questions, concerns, and issues. Being a Outpatient Care Coordinator facilitates collaboration with multi-disciplinary teams to make recommendations for improvements to the patient experience. Ensures that escalated problems are addressed and resolved. Additionally, Outpatient Care Coordinator provides and maintains up-to-date informational materials and resources for patients and families. Follows all applicable regulations regarding patient information privacy policies. May require an associate degree. Typically reports to a manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members.     JOB TITLE: Care Coordinator

    EMPLOYMENT TYPE: Full-time, 40 hours/wk

    SCHEDULED HOURS: Monday-Friday, 8AM- 4:30PM

    PROGRAM/LOCATION: Adult Outpatient Program, Willimantic, CT

    PC#: 1520

    ABOUT THE PROGRAM:

    The Adult Outpatient services provide psychiatric & substance abuse diagnostic evaluation and treatment. Services include assessment, consultation, individual, couples, group, family treatment, and medication evaluation and monitoring

    Duties & Responsibilities:

    • Coordinate intakes and services for clients. Track waiting lists for services.
    • Insure client activity data: attendance, absence, drug screens, med management, etc., completed and entered into the CDCS database accurately and within the contractually established time frames.
    • Maintain careful documentation of contacts and communication to maintain quality services.
    • Work as a liaison between agencies, to coordinate care for referred clients.
    • Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payeeship, legal services, medical services, education, and employment.
    • Assist with scheduling and providing transportation to and from appointments.
    • Maintains documentation in compliance with agency and program standards.
    • Participates in staff meetings, supervision
    • Additional duties as required.

    QUALIFICATIONS:

    Education: Bachelor’s Degree in Behavioral Health or 3 years relevant experience required.

    Experience: Excellent communication skills. Excellent computer skills. Experience working with individuals in substance abuse treatment who may also have co-occurring mental health disorders is preferred. Minimum 2 years experience in the field of care coordination preferred.

    Licensure/Certification/Registration: Valid driver’s license is required.

            COMPENSATION*:

            $18.86 /hour min.-$28.29/hour max., dependent upon experience.

            • Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
