Outpatient Care Coordinator supervises the day-to-day activities of the patient relations team of a healthcare organization to deliver patient liaison services and patient-centric care. Coaches staff on best practices for responding to patient and family questions, concerns, and issues. Being a Outpatient Care Coordinator facilitates collaboration with multi-disciplinary teams to make recommendations for improvements to the patient experience. Ensures that escalated problems are addressed and resolved. Additionally, Outpatient Care Coordinator provides and maintains up-to-date informational materials and resources for patients and families. Follows all applicable regulations regarding patient information privacy policies. May require an associate degree. Typically reports to a manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members.     POSITION OVERVIEW

    The Care Coordinator identifies patients with preventive and chronic condition care gaps and performs patient outreach to close gaps. They work with patients to identify barriers to care, offers assistance to help reduce identified barriers, as well as schedule and track referrals for specialty care.

    SCHEDULE AND PAY

    This is a full-time (40 hour) position.

    Schedule: Monday – Friday: 8:00 AM- 5:00 PM; Pay Rate: $19-21

    DUTIES

    • Care gap duties:
      • Uses Electronic Health Record (EHR) to identify patients due for preventive or chronic care services (care gaps) based on approved protocol/guidelines.
      • Conducts patient outreach through phone calls, letters, to inform and encourage patients to close identified care gaps.
      • Works with patients to identify barriers to care and helps identify resources to address barriers (e.g. transportation, ).
      • Communicates information appropriately with care teams related to patients’ needs.
      • Reviews EHR for patients with care gaps to identify previous screenings and data enter results, if needed.
      • Works with Care Management Supervisor & QI Director to establish priority clinical quality measures, to set weekly/monthly goals and collects data to monitor progress of established goals.
    • Referral coordination duties:
      • Schedules patients for specialty referrals and diagnostic studies.
      • Processes incoming and outgoing referrals through the UniteUs portal.
      • Requests records from specialists when referral appointments have been completed.
      • Works with patients to identify & address barriers to attending referral appointments.
      • Tracks referrals through to completion and documents status in EHR.
    • Participates in quality related workgroups, as assigned. Actively participates in Colorectal Cancer Screening Workgroup and committee projects, as assigned. Assists other ACHC team members related to preventative & chronic care services and social determinants of health.

    QUALIFICATIONS

    • Certificate or diploma from an accredited Nursing Assistant (NA), Medical Assistant (MA) or Licensed Practical Nurse (LPN) program.
    • Experience in a community health center preferred.
    • Experience in case management.
