    This position is located in our brand-new Oakland City Center location, or remotely (within the state of California). We are flexible provided you are open to traveling to the Oakland office locations as needed.

    About Your Work

    As the RN Lead, you will be a subject matter expert and the first level escalation for the Outpatient Care Management team. The Clinical Lead participates in special projects and workgroups at the direction of the department manager. This individual possesses excellent verbal and written communication skills. The Lead LVN CM maintains a small caseload and is responsible for SNP Complex Case Management and Complex Case Management activities including Transition of Care.

    You will focus on:

    • Clinical decision support of Outpatient team members, by training, auditing, assisting with complex case review and decision making.
    • Provide case management activities across the continuum of care in order to facilitate and promote high-quality, cost-effective outcomes.
    • Assist in the development, implementation and evaluation of care standards, policies, procedures and protocols to improve Care Management across the care continuum.
    • Participate in employee training/orientation and mentoring of team members as indicated.
    • Assigns and or covers for Care manager PTO/open positions.
    • Research issues to facilitate objective medical management decisions based upon approved criteria, standards and guidelines

    The Skills, Experience & Education You Bring

    • Demonstrates strong ability to effectively apply, interpret, and communicate policies, procedures, clinical guidelines, medical policy, regulations, and standards.
    • Demonstrates excellent verbal and written communication skills.
    • Ability to read and understand medical records and extract data pertinent to the creation of a transition of care/case management care plan.
    • Demonstrates strong ability to effectively cope with stressful situations, manage multiple, and sometimes conflicting priorities simultaneously
    • Demonstrates strong ability in educating and training clinical and non-clinical staff.
    • Demonstrates strong working knowledge of managed care concepts, Medicare/Medi-Cal Guidelines.
    • Demonstrates working knowledge in Microsoft Suites.
    • Required: LVN Degree
    • Preferred: Bachelor of Science in Nursing or Master of Science in Nursing
    • Preferred: Five (5) or more years recent Case Management leadership experience in a Health Plan, IPA or Managed Care Setting
    • Preferred: 3 years’ experience in EPIC software

    The anticipated pay range for this role is listed in our salary posting for transparency but may vary based on factors including the candidate’s qualifications, skills, experience, and geographic location. This base pay range is specific to California and can vary based on Bay Area, Metro LA, and Greater California regions which may not be applicable to other locations.

    Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
