Summary
Responsible for the day to day coordination of services provided by the Home Care Services therapy and rehab clinicians. Is accountable for promoting efficient quality patient care in the patient’s place of residence. Plans, organizes and directs agency activity and assists with supervisory on-call responsibilities. Works closely with the Director and Clinical Manager on agency activities and serves as a resource, educator and role model to aid in achievement of high quality care for our patients. Provides direct patient care as needed.
Position Reports To: Home Health and Hospice Director
Essential Functions
- Collaborates with the Director/Clinical Manager to determine staffing needs for the department.
- Works closely with the Intake/Referral Coordinator to effectively schedule patients based on geography, skill level and patient need.
- Supports and effectively utilizes staff members' technical specialties and skills.
- Provides Supervisory on-call duties on a rotational basis.
- Provides teaching and skills development through one-on-one interaction with therapy/rehab staff during the delivery of direct patient care.
- Role models performance expected when making patient visits.
- Schedules new staff and review documentation related to regulatory OASIS standards to improve outcomes and appropriate case mix index.
- Orients and supervises new clinical staff during their orientation process.
- Organizes and participates in educational programs.
- Participates in the development of clinical staff skills; assists the Director in developing, presenting and implementing changes in agency practice.
- Attends workshops and seminars in order to update knowledge. Reports significant conference findings to staff.
- Maintains a high standard of professional responsibility and performance.
- Develops policies, procedures and standards for the Home Care Department.
- Collaborates with Director, Clinical Manager and Quality Specialist to develop annual unit-specific goals and objectives.
- Works with support services and community resources to achieve high quality of care for patients and their families.
- Applies appropriate current theoretical concepts to implement changes in practice within the agency.
- Demonstrates professional commitment through membership in committees, institutional activities, specialty organizations and active participation on quality lead team.
- Researches and resolves Variance Reporting as requested by the Director.
- Responsible for yearly evaluations of therapy/rehab staff and completes the supervisory visits/evaluations of these clinicians. Participates in the disciplinary process of staff with assistance of the Director/Clinical Manager.
- Participates in the interview and hiring process for new staff.
- Assists the Director and Clinical Manager with meeting budgetary compliance as evidenced by monitoring and enforcing productivity standards of clinical staff, minimizing overtime and problem-solving with clinical staff on day-to-day workflow.
- Required to maintain active Basic Life Support (CPR) certification.
- Other duties as assigned.

Qualifications
Required:
- Graduate of an AOTA accredited occupational therapy program, BS degree from an CAPTE accredited school, or Masters degree in Speech Language Pathology/ certification of clinical competence by the American Speech and Hearing Association.
- Current License with State of Washington as an Occupational Therapist, Physical Therapist, or Speech Language Pathologist.
- Minimum of four years’ experience working in a Home Care Setting.
- Demonstrated leadership and supervisory skills.
- Demonstrated teaching abilities.
- Communication: Functionally able to listen, read, speak and write using English language. Functional hearing and visual acuity sufficient to detect changes in patient conditions and perform assessments. Effective interpersonal skills. Demonstrates ability to work in collaboration with health team members.
- Must possess basic computer/electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication and password management.
- Current, valid driver's license and access to an insured vehicle with the minimum coverage by law.
Desired:
- Past leadership experience in the home care, acute care or skilled nursing setting.
- Experience with managing schedules and employee relations.
- Past experience in process improvement activities.

Working Conditions:
Many of the essential job functions of this position are done while seated at a desk. This may be in an open hallway or in a more confined space. Much work is done independently but there is also interaction with others, both in person and by phone. Overtime hours may occasionally be required. There is mental stress and pressure associated with this position. General supervision is provided by the department Practice Manager with additional instructions, guidance, and directions provided by the Department Supervisor, physicians and clinical staff.
