Job Summary:
We are seeking a dedicated and compassionate Registered Nurse to join our Primary Care Practice. This position is a hybrid position focused on both primary care and mental health. Your primary function will be to serve as a behavioral healthcare manager for our CoCM Program which allows us to integrate Psychiatric Care into Primary Care. CoCM Is a new program that CMS has implemented which utilizes a behavioral health manager to coordinate with a psychiatric consultant, the PCP, and the patient to ensure that patient’s mental health needs are being met. CoCM tasks are remote and will include both indirect asynchronous work as well as direct patient telecommunication. While completing tasks for CoCM you will be stationed at one of our PCP clinics to assist with tasks such as welcoming patients, vitals checks and occasional injections or simple procedures. Your primary responsibility however will be to attend to the needs of the CoCM patient panel.
CoCM Responsibilities:
-Manage patient panel and maintain updated consents.
- Perform routine checks and symptom screenings
- Ensure medication adherence and coordinate need for medication refills
-Ensure routine follow ups are scheduled with internal or external psychiatric provider.
-Assist with patient resource coordination
-Communicate with PCP and psychiatric consultant as required

Primary Care Responsibilities:
-Ensure new patient packets are complete and uploaded
- Monitor and document patients' vital signs upon intake
-Assist with simple medical procedures such as injections, nebulizer treatments, UA, ECG, etc.
