*FULL TIME RN for 2 OUTPATIENT CLINICS (COLUMBIA AND GREENE COUNTY) AND 2 RESIDENTIAL HOUSES.
Primary Purpose:
Responsible for obtaining medical history and physical examinations for Outpatient Clinic clients at the Columbia County and Greene County Clinics.
Duties and Responsibilities:
1. Meets with each new client to complete the Outpatient Clinic Medical Screening document to establish the need for a physical examination and assess the client’s available medical history status as required by Part 822 of O.A.S.A.S. regulations.
2. Obtains copies of client’s available medical history and physical examination documentation.
3. Places and reads PPDs for clients and employees
4. Takes vital signs of clients as ordered by doctor or as needed
5. Administers medication to clients as needed and as directed by supervising physician
6. Monitor clients, including administering Urine Drug Screens on all newly admitted clients and any other clients as needed
7. Maintains schedule for all doctor visits for each client
8. Requests orders for all mediations that need new prescriptions
9. Prepares letters for clients who need a complete physical examination if a client does not have an available medical history and physical examination that was performed within six (6) months of the client’s admission.
10. Assures that all medical history and physical examination documentation includes screenings required by Part 822 of the O.A.S.A.S. regulations.
11. Maintains accurate medical records for each client in the electronic health record to include medical history and physical examination documents.
12. Makes appointments for additional screenings if current medical history and physical examination documentation does not meet Part 822 of the O.A.S.A.S. regulations.
13. Assumes responsibility for reporting medical incidents to Program Manager.
14. Make calls to local ambulance service, fire department, police, or CDPC, as needed.
15. Attend training programs as appropriate.
16. Attends staff meetings when assigned to attend by the Program Director.
17. May perform other duties as assigned by the Program Director or the Clinical Supervisor.
18. Serves as Health Coordinator for the agency.
You will be responsible for nursing duties at our 2 clinics, Col and Greene as well as our 2 residential programs, Red Door - Hudson and Riverside - Catskill.
