A unique position for a skilled personal support worker providing direct support in a 1 : 1 environment with a young adult client with down syndrome.
Personal support worker’s role includes direct care and coordinated supports and services for one individual living in their own residence in Evanston, IL seeking to live as independently as possible with support.
The personal support worker is in-person and responsible for ensuring the delivery of services and supports across settings in order to enhance the well-being, quality of life, opportunities and achievement of personal goals for one person with developmental disabilities.
New hires would need to be fully vaccinated with 1 or 2 doses and booster and provide their card upon hire OR begin receiving the booster within 2 weeks of being hired and participate in weekly COVID testing until fully vaccinated OR provide documentation for an ADA or religious exemption and participate in weekly COVID testing.
