Job Overview

A unique position for a skilled personal support worker providing direct support in a 1 : 1 environment with a young adult client with down syndrome.

Personal support worker’s role includes direct care and coordinated supports and services for one individual living in their own residence in Evanston, IL seeking to live as independently as possible with support.

The personal support worker is in-person and responsible for ensuring the delivery of services and supports across settings in order to enhance the well-being, quality of life, opportunities and achievement of personal goals for one person with developmental disabilities.

• Essential Job Functions
• Weekend Personal Support Worker
• Part Time to start Mid-May.
• 8 hour shift - variable based on client’s activity schedule.
• Day / early evening shift.
• 2 weekends per month (weekends vary, depending on client’s schedule).
• Hourly rate based on experience level (range $16-$20 per hour).
• Plan fun, engaging, and appropriate weekend community activities and outings.
• Serve as an advocate on behalf of the individual.
• Provide direction, scheduling and in person support.
• Support and participate in clients' creative interests / pursuits.
• Work with the client to plan and prepare healthy meals and participate in clean up tasks.
• Work with client to maintain a clean living environment and provide basic house cleaning tasks.
• Work with the client to maintain and update personal calendar including assisting in planning social opportunities.
• Provide and / or monitor transportation with a well maintained vehicle and clean driving record to / from community programs and activities with mileage reimbursement.
• Communicate with parents / guardians and Care Manager regularly to maintain positive relationships and to ensure that service and support are implemented and continue to meet the individuals desired outcomes and needs.
• Seek opportunities for community engagement and communicate them to the Life Care Manager.
• Meet the guardian’s requests and client’s individual needs.
• Monitor individual’s overall progress; communicate supports and outcomes to the parents / guardians and the Life Care Manager.
• Requirements
• Minimum of one year of non-binding commitment.
• High School Diploma Required. Bachelor’s Degree is preferred.
• Minimum of one year experience working with individuals with intellectual / developmental disabilities.
• First Aid / CPR Training Certificate.
• Ability to effectively interact, collaborate, and communicate with individuals at all levels; proven strong written and oral communication skills are a must.
• Excellent planning, self-management and organizational skills are crucial.
• Ability to teach skills of independent living and work in tandem with the individual while providing safety oversight.
• Effectively communicates and reports outcomes with parents / guardians and Life Care Manager weekly.
• Appreciation for the arts and related fields.
• Required to have fingerprinting as part of a background check process.

New hires would need to be fully vaccinated with 1 or 2 doses and booster and provide their card upon hire OR begin receiving the booster within 2 weeks of being hired and participate in weekly COVID testing until fully vaccinated OR provide documentation for an ADA or religious exemption and participate in weekly COVID testing.
