Pastoral Care Director - Healthcare develops programs and oversees services designed to meet the religious and spiritual needs of patients, their families, and healthcare staff. Ensures that pastoral services are staffed so that chaplains are available to support patients and families facing difficult medical choices. Being a Pastoral Care Director - Healthcare establishes policies and procedures to facilitate optimal service delivery and ensure that emotional support, spiritual counsel, and comfort are provided. Evaluates and reviews records of counseling sessions and visitation logs to ensure compliance with professional and ethical standards. Additionally, Pastoral Care Director - Healthcare plans and coordinates with other clergy to conduct interfaith worship experiences. May perform the duties of a chaplain. Requires a master's degree. Typically reports to senior management. The Pastoral Care Director - Healthcare typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a Pastoral Care Director - Healthcare typically requires 3+ years of managerial experience.     POSITION SUMMARY

    The VA Account Specialist supports and assists the VA Account Director in transitioning as well as preparing patients to receive services from Pinnacle Home Care. The VA Account Specialist is responsible for organizing, tracking, and following up on the transition of patients from the VA to Pinnacle Home Care.

    RESPONSIBILITIES

    Responsible for the tracking, following up and requesting of additional documentation for new referrals to include additional orders, hospital holds, locating patients for Starts of Care, labs, additional visits, skills, durable medical equipment requests, expiring authorizations as well as requesting new orders.

    Responsible for the tracking, follow up and requesting of additional documentation for Resumption Of Care orders to include:

    • Confirm branch has uploaded ROC orders to patient chart in EMR system.
    • Confirm dates for ROC.
    • Confirming the correct physician has been assigned to orders.
    • Facilitate resumption of care cases by providing appropriate notification to the branch of pending discharge and ROC orders.
    • Complete welcome calls to introduce the veterans to Pinnacle Home Care.
    • Assist branches with any follow-up questions or other help needed to provide SOC support.
    • Continue to monitor and maintain existing patient concerns.
    • Provide timely notification to Central Operations and Branch if any corrections are needed regarding payor source, region, or physician in accordance with existing SOP’s.
    • Assisting branch to contact patients.
    • Exhaust all efforts to locate the patient prior to making the patient a non-admit.
    • Confirm receipt and initiation of orders.
    • Exercises clear and consistent communication with branch and VA Account Manager regarding patient needs.
    • Confirming DME requests with branch and VA Account Manager.
    • Additional requirements as needed.

    QUALIFICATIONS

    • Must possess excellent prioritization and time management skills.
    • Must be able to communicate effectively and clearly.
    • Basic computer competencies with Excel, Word, and other EMR systems.
    • This position requires that the employee is willing and able to meet the physical requirements as necessary.
