The Nursing & Rehabilitation Center is a 5-star, 460-bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, a ventilator dependent care, palliative and hospice care, a new Memory Care Unit and on-site dialysis. The Gurwin Family of Healthcare Services also includes the 201-apartment Gurwin Jewish~ Fay J. Lindner Residences assisted living community, and the proposed Fountaingate Gardens independent living community, to be built on the Commack campus.

Our Assisted Living Community is currently seeking a full time LPN to work as the Assistant Director of Enriched Pathways (our memory care wing) every Monday-Friday, 8:30am-5:00pm. Our ideal candidate will embody the passion and commitment required to deliver the ultimate care for our residents. The Director of Enriched Pathways at Gurwin Jewish ~ Fay J. Lindner Residences must be innovative, creative, dynamic, and strategic to continue our reputation of providing the gold standard of excellence in assisted living.

Responsibilities include but are not limited to:

• Overall responsibility for our Memory Care Wing. Supervise all personal care services and Recreation programs rendered by Gurwin Jewish ~ Fay J. Lindner Residences.
• Setting individualized service plan goals (with Case Manager) for the residents and assisting them with achieving those goals.
• Establishes policies and procedures and all standards of care for our residents.
• Coordinate and manage all Assistive Living Aides in Enriched Pathways.
• Review’s and establishes (when necessary) all Department of Health required forms during the admission process.
• Responsible for personnel in the department with respect to hiring, terminating, counseling and performance evaluation.
• Responsible for employee education to ensure staff excellence with respect to policies, procedures and all standards of care.
• Apprise family of changes in resident health and behavior.
• Responsible to ensure residents receive all necessary medical services including medication administration, physician services and any necessary ancillary services.
• Responsible for keeping the Administrator informed regarding the department and residents.
• Supervise daily logs and verifying that they are being maintained by all shifts.
• Develop new initiatives, programs, and cost effective measures to bring the Community to the next level.
• Actively participates in the prevention of employee incidents and accidents. Makes recommendations to improve safety of employees and residents.

Requirements include:

• Preferred Candidate will be a Licensed Professional Nurse with current NYS license.
• Previous leadership experience in a specialized dementia program.
• Must be knowledgeable and experienced in caring for a person with dementia, the disease process, the changes associated with a demanding illness and how they affect an individual’s ability to function.
• Must be knowledgeable in Department of Health Regulations as they pertain to Assisted Living Facilities.
• Must have prior supervisory experience.
• Must possess excellent analytical skills and be detail orientated.
• Must have exceptional interpersonal skills, coupled with strong relationship building and be an effective leader who can motivate and manage staff members.
• Committed to the well-being of our residents and demonstrates compassion and respect for their concerns.
• Previous Assisted Living experience is strongly preferred.
