Job Description:

As the Director or Nursing you will be in charge of a team and help to provide supervisory visits for clients. Our ideal candidate has 2 plus years of nursing experience (bonus if in home caregiving), Georgia Medicaid waiver program experience, and willing to work in our Sandy Springs office and commute to the patients’ homes when necessary.

Schedule: Full Time- Monday to Friday

Location: Sandy Springs, Ga and surrounding areas

Responsibilities:

• Works to provide an assessment of the needs of the SFC and PSSX clients and caregivers and creates a coaching plan
• Monitors and provides ongoing assessments, training, and support to the primary (family) caregiver and the client, adjusting as necessary.
• Provides initial, modified, and annual reassessments to ensure the Family Caregivers’ goals and needs are being met.
• Performs supervisory visits as needed per program guidelines
• Analyzes patient documentation received from case management; builds out and amends care plan on an ongoing basis
• Travels to complete all in home requirements as needed.
• Travel outside of Metro Atlanta home region on an as needed basis
• Maintains clinically accurate records within corporate CRM
• Advises case management re: any and all changes to clients’ condition, status and diagnosis
• Assists other department RNs and health coaches with process, cross training and completing tasks, as necessary
• Reviews and co-signs all documentation regarding LPN’s supervisory visits within 10 days of said visit
• Assists in recruitment of additional staff, i.e RN’s, LPNs, health coaches
• Markets company’s services and increases client base on a consistent basis
• Performs any additional job functions within current role, on an as needed basis

Requirements

• GA RN License
• At least two years of experience with elderly or frail people.
• Knowledge of GA Medicaid Waiver programs and home and community-based resources is a plus.
• Experience with home health a plus
• able to work in an environment independently and identify areas for improvement and work towards those improvements independently.
• Takes initiative
• Detail oriented
