Must have the following updated and original documents for consideration:

• State ID
• Level II Background Check
• Social Security Card
• Proof of Residency/Citizenship
• Professional Liability Insurance
• Physical Exam
• CPR Card
• CNA License or HHA Training Certificate
• HIV/Aids Certificate
• Abuse, Neglect and Exploitation Certificate
• Assisting with Self-Administered Medication Certificate
• Alzheimer's/Dementia Certificate
