We have a great opportunity for an experienced RN/Director of Nursing to run the day to day operations of the office, as well as temporarily case manage in the field, as eventually, this role will be solely office-based as the Director of Nursing.

JOB SUMMARY for dual role:
Responsibilities of the Director of Nursing include:
- Planning, coordinating, delivering, and overseeing the delivery and outcomes of required healthcare-related services/training programs on a routine basis
- Monitoring both skilled and non-skilled healthcare services
- Conducting performance monitoring, identifying and reducing risks
- Demonstrating fiscal accountability by monitoring operations to ensure compliance with ResCare policies and procedures, state licensure and regulations, Medicaid and Medicare standards, as well as state reimbursement regulations
- Facilitating communication and serving as a liaison between and among nursing and direct care staff, family and/or guardians, and community medical providers and professional consultants in cooperation with operations management
- Directly overseeing initial start of healthcare, assessment, and first home visit (or comparable) of all new contracts with clinical component
- Directly supervising the implementation, coordination, maintenance, evaluation, and termination of care plan and services, analyses outcomes, OASIS and ICD9/10, medical records, and physician orders
RESPONSIBILITIES for RN Case Manager:
- Perform the initial home care patient visit and re-evaluate the patient's needs and progress on a regular basis.
- Initiate the plan of care under doctor's orders.
- Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patients.
- Observe, assess, and document symptoms.
- Monitor reactions and patient progress.
- Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
- Educate patients and caregiver on techniques for in home health care.
- Coordinate patient services.
- Supervise LPN's and HHA's.
- Notify the physician and other personnel (DON, PT, Case Manager) of change in the patient's condition.
- Perform skill outline in the agency's approved policy and procedure manual.
- Discharge the patient from skilled nursing services when the discharge criteria have been met.
- Case conference clinicians providing care to ensure coordination of care.
- Update clinical records according to policy and procedures.
- Attend in-service programs, continuing education programs, seminars, and self-study programs annually.
- Provide onsite supervision of LPN/HHA.
- Adhere to Medicaid and Medicare regulations.

Additional requirements for the dual role include:
- Bachelor’s Degree in Nursing
- Graduate of an accredited school/college with current registration of practice as a Registered Professional Nurse
- Excellent communicator with ability to handle concurrent tasks and responsibilities Demonstrate proficient skills using Oasis to admit, transfer, re-certs, and discharge home health patients in regards to physician orders. Coordinate and supervise LPN's and HHA's in the delivery of patient care. Maintain compliance with agency policy and procedures. Follow Medicare and Medicaid regulations.
- Registered Nurse (RN)/current license.
- Minimum of 3 years of Home Health nursing experience as an RN.
- Work positively and favorably with patients, families, and staff.
- Must have own vehicle, auto insurance and driver's license.
- Experience with Oasis/Start of Care paperwork (initial visit)
