The Patient Care Coordinator (PCC) manages hospitalized patients from admissions to discharge in collaboration with the care team assigned. This individual oversees clinical guidelines, assessing appropriateness and outcomes of care; develops interdisciplinary, individualized plan of care for each patient assigned, utilizing a scientific problem solving approach and in collaboration with the patient, family, physician, and other care team members; coordinates the discharge process to promote continuity of care and optimal outcomes; makes timely referrals to various disciplines; and facilitates patient and family education. The PCC is the leader of the core care team, providing direction and ongoing development to team members. The PCC has direct patient contact, access to confidential patient and organizational information, and functions independently in collaboration with other care team members and under the direct supervision of the Director.
