    Position: Intake Patient Coordinator – Hospice and Palliative Care

    Location: 1480 Fowler St Richland, WA 99352

    Type: Full Time, Non-Exempt

    Anticipated Schedule: 8 am to 4:30 pm (30-minute lunch) or 8 am to 5 pm (1-hour lunch) 5 days a week with rotating weekends and holidays. Must be available to work on-site with a hybrid onsite and telework opportunity after a 90-day Introductory Period.


    Job Summary:


     Join our compassionate team at Tri-Cities Chaplaincy as an Intake Patient Coordinator specializing in Hospice and Palliative Care. Your role will be crucial in facilitating the admission process for patients entering hospice and palliative care services. The Intake Patient Coordinator performs a variety of administrative support duties associated with the hospice intake and/or referral process following internal processes and procedures. Additionally, the Intake Patient Coordinator acts as a knowledgeable resource, offering essential information to families considering hospice and palliative care options.

    Responsibilities Include but are not limited to:

    • Referral Processing: Accepting referrals, entering demographic and insurance data into electronic medical records, requesting additional records and information s needed, verifying insurance coverage, and obtaining prior authorization of services. 
    • Administrative Support: Answering phone calls, assembling admission packets and preparing charts for admission, ordering durable medical equipment (DME) as directed by the nurse, verifying all necessary documents/orders are received, maintaining filing systems, and handling various administrative tasks to maintain compliance with Medicare and Medicaid programs. 
    • Resource and Knowledge Sharing: Act as a knowledgeable resource, offering essential information to families considering hospice and palliative care options. Provide guidance on available services, answer questions about the intake process, and ensure families have the information they need to make informed decisions.

    Compensation


    Starting Range: $17.67/hour - $22.53/hour

    Pay Range: $17.67/hour - $27.39/hour

    · Mileage Reimbursement: $0.67 per mile. – for eligible positions. 

    · Vacation: Accrual of hours each pay period based on hours worked.

    · Annual "Yay Day": Full-time employees receive 1 paid day off every January to volunteer in the community.

    Requirements

      

    Qualifications:


    • High School Diploma or Equivalent 
    • Two (2) years of related work experience in a medical office, healthcare facility, hospice, or palliative care setting.
    • Two (2) years of experience in medical terminology and abbreviations.
    •  Strong communication and interpersonal skills, with the ability to interact compassionately with patients and families.
    • Excellent organizational and time management skills, with attention to detail.
    • Ability to work effectively in a fast-paced environment.
    • Proficiency in computer applications, Microsoft Office, and electronic medical records (EMR) systems.
    • Available to work weekends and holidays as part of a rotating schedule.
    • Available to work occasional overtime. 
    • Strongly detail-oriented with good memory recall. 
    • Ability to write concisely, and legibly. 
    • Ability to follow written and verbal instructions.

    AGENCY REQUIREMENTS

    • Participate in community Education and Outreach to raise awareness of agency-provided programs including but not limited to hospice care, advanced care planning, and bereavement.
    • Responsible for knowledge and compliance with agency policies, procedures, and practices.
    • Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a “need to know” basis only.
    • Reliable personal transportation, and current and valid driver’s license, and vehicle insurance.
    • Satisfaction of state and agency requirements for Tuberculin screening.
    • Hepatitis B immunization series, proof of series, or signed declination.
    • Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
    • Must pass criminal background screening and pre-employment drug screening.
    • Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
    • Complete agency and departmental orientation and training program(s).
    • Complete all continued learning, education, policy reviews, and required training by their respective deadlines.
    • Maintain licensure(s) and certification(s) required for the position.
    • Available to work as assigned/scheduled.
