Patient Safety Manager manages a staff responsible for developing and implementing patient safety programs. Supervises the investigation of errors and then plans for resolution of all safety related incidents, including those involving employees, visitors, patients, and situations posing a threat to the hospital's buildings and equipment. Being a Patient Safety Manager maintains records of patient-related errors and evaluates this information in order to identify the cause of the error. Collects, reviews, and analyzes information from the facility's safety programs to identify corrective action and programs as necessary. Additionally, Patient Safety Manager recommends changes, policies, or programs that could prevent future errors. Designs and presents training programs meant to increase awareness of patient safety initiatives. Ensures compliance with state and federal regulatory requirements. Requires a bachelor's degree. Typically reports to a head of a unit/department. The Patient Safety Manager manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. Extensive knowledge of department processes. To be a Patient Safety Manager typically requires 5 years experience in the related area as an individual contributor. 1 to 3 years supervisory experience may be required.     The Patient Safety Specialist will assist in developing, implementing, analyzing, coordinating, and evaluating patient safety events, systems, and processes. Coordinates performance improvement and patient safety initiatives to support a culture of safety and reduce health care errors and risks. This may include participation in data analysis, chart reviews, root cause analysis, focus studies specific to patient safety, staff/patient education, patient safety rounding and observations. The Patient Safety Specialist will function as an expert and educator on relevant patient safety initiatives and will lead or participate in multidisciplinary projects and improvement teams.

    Overview of Responsibilities

  • Under the direction of the Director of Quality supports by analyzing and coordinating patient safety activities and initiatives.
  • Works to encourage and support a culture of safety in the organization.
  • Investigates and evaluates events for risk potential, loss minimization, mitigation, and proper documentation.
  • Assists with the coordination and communication patient safety initiatives.
  • Provides consultation and support to teams and departments related to performance improvement initiatives and patient safety events.
  • Assists in managing data, identifying trends, preparing graphs and power point slides.
  • Collaborates professionally with staff, managers, and providers in addressing and resolving patient safety issues.
  • Participates in root cause analysis (RCA), failure mode and effects analysis (FMEA), and cluster investigations.

    Education/Experience

    Education: Bachelor’s degree in nursing or other related field

    Experience Required: Three years’ experience in acute care hospital 

    Licensure and Certification: Certified Professional in Patient Safety (CCPS) beneficial

    Experience Prefered:

    • Experience in FMEA and RCA processes.
    • Familiarity with change management.
    • Ability to formulate and lead collaborative teams.
    • Excellent verbal and writing skills.
    • Patient Safety certification preferred.
    • Knowledge of computer programs, MS Office products and EMR.

    Schedule Details

    Day Shift. Monday-Friday, 8:00am-4:30pm.
