Patient Safety Specialist assists in the development and implementation of patient safety programs. Gathers data detailing patient-related errors and conducts analysis for management detailing the cause of the error. Being a Patient Safety Specialist recommends changes, policies, or programs that could prevent future errors. Gives presentations and training programs meant to increase awareness of patient safety initiatives. Additionally, Patient Safety Specialist tests patient-safety knowledge of employees and recommends changes to training programs. Requires a bachelor's degree. Typically reports to a manager or head of a unit/department. The Patient Safety Specialist contributes to moderately complex aspects of a project.     ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES

    • Evaluates, identifies, plans, coordinates, and leads system-wide patient safety activities to prevent patient harm. Reporting directly to the Director of Quality Assessment Performance Improvement, the Patient Safety Specialist will work closely and collaboratively with identified internal and external customers to facilitate patient safety initiatives.
    • Develops and implements a Patient Safety Plan and patient safety evaluation system, program will be reviewed on an annual basis.
    • Determines risk for exposure of harm/potential harm to patients in response to identified opportunities, outcomes, etc. Assists in the determination of the level of containment/control required for: immediate harm, risk for harm in current process and future potential for harm.
    • Identifies patient safety opportunities. Coordinates, investigates, analyzes, and responds to quality/patient safety issues, events, and significant care issues or trends. Conducts surveillance, analysis, and data reporting for patient safety, while complying with regulatory/accreditation requirements.
    • Oversees the management and use of provision of care, patient injuries and falls and Root Cause Analysis (RCA) and RL6 event reporting system.
    • Assesses/investigates patient safety-related events. Facilitates root cause analysis, proactive risk assessments, and variation reduction; and identifies improvements related to patient safety and prevention of harm through the use of process improvement methodologies and tools.
    • Leverages lessons learned and communicates recommendations/required actions to avoid future exposure of harm to patients. Presents data and reports deficiencies identified during internal surveillance and monitoring, and the status of implementation to key stakeholders and the Quality Services Committee.
    • Uses evidence-based patient safety methodology to support, develop and implement initiatives across the organization in conjunction with the Director of Quality Assessment Performance Improvement and Chief of Quality.
    • Reviews event reporting system and analyzes entries to address patient safety concerns and identify trends actively collaborates with involved teams and/or departments.
    • Serves as a subject matter expert on internal and external patient safety matters. Collaborates with system and facility teams: care management teams, clinical discipline teams, facility leadership, risk management, and other groups as appropriate. Provides education and resources to address changes, identify gaps in compliance, and educate leaders and staff on patient safety requirements. Assists when requested in the collection of patient related incident information.
    • Conducts testing on employees regarding patient-safety and recommend changes to training programs
    • Provides support to internal and external customers to further the culture of safety.
    • Manages the Just Culture program and supports non-punitive error reporting.
    • Ensuring security and privacy of data in all our systems.
    • Performs other duties as assigned.

    MANDATORY MINIMUM QUALIFICATIONS

    Experience: Three (3) years of direct experience in healthcare quality or risk management or equivalent.

    Education: Bachelor of Science Nursing degree.
