    FLSA Status: Non-Exempt

    Department: Nursing - HHC Reports to: Manager of Home Health Services


    Position summary: Primarily responsible for providing assistance with transportation to medical appointments for the purpose of promoting and maintaining the maximum level of independence for each resident.


    Works cooperatively and respectfully with residents, coworkers, families, visitors, and volunteers to create a positive and pleasant environment in the home. Promotes the residents' psychosocial well-being through meaningful relationship building, and works with the household team to meet other social service needs of the resident and of the household, always providing support, friendship and kindness for each of the household members. All tasks are performed in accordance with current standards, policies, guidelines and regulations and in a safe and efficient manner.


    Essential skill and experience:

    • Nursing Assistant Certification in Tennessee, or be eligible to obtain certification within four months of employment.
    • Time management: the ability to organize and manage multiple priorities.
    • Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations, and communicative skills; auditory and visual skills; satisfactory completion of post-offer, pre-employment physical as required.
    • Valid TN State Drivers License with a F Endorsement and a safe driving record
    • Operate a lift vehicle

    Preferred skill and experience:

    • Computer proficiency, including Word and Excel

    Reporting to this position: No direct reports.


    Essential Duties:

    • Escorts residents to medical appointments and retrieves residents written orders or written plan of care from their medical doctor.
    • Obtains vital signs as needed, such as B/P, P, R, T.
    • Health maintenance and monitoring tasks:
    • Observes and reports changes in residents’ physical, mental, emotional status to nurse.
    • Observes, reports, and documents resident status and the care and services furnished.
    • Monitors for environmental safety events.
    • Contributes to the review and revision of resident care based on on-going assessments, changes in health status as resident condition warrants.
    • Emergency preparedness:
    • Recognizes medical emergencies and responds appropriately, notifying nurse.
    • Responds to and acts appropriately in emergency and/or disaster situations.
    • When not transporting, perform any other duties as assigned including answer phones at HHC.
    • Assists with personal care for Village residents.
    • Daily schedule may vary according to transports scheduled.
    • Performs other related duties as assigned by Home Health Care Nurse and/or Manager of Home Health Services.

    Essential Expectations:

    • Must coordinate with the nursing team and provide resident/patients with nursing care as outlined in the department’s policies and procedures.
    • The employee is expected to adhere to all Uplands policies.
    • Maintain the comfort, privacy, and dignity of each resident in the delivery of services to them. Interact with residents in a manner that displays warmth and promotes a caring environment.
    • Fully understand all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the nurse, director of health services, or administrator incidents or evidence of resident abuse or violation of residents’ rights.
    • Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items.
    • Perform all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves, and proper disposal of soiled materials.
