Typically reports to a supervisor. The Personnel Records Clerk works under the close direction of senior personnel in the functional area.     Position Summary:
    The HIM Clerk plays a vital role in ensuring the accuracy, confidentiality, and accessibility of patient health information within the hospital. This position involves various administrative tasks related to health information management, including data entry, record maintenance, and compliance with regulatory standards.

    Essential Functions:

    1. Perform      data entry tasks to accurately record patient information into the      electronic health record (EHR) system.
    2. Retrieve      and process medical records requests from healthcare providers, patients,      and other authorized personnel in a timely manner.
    3. Maintain      the integrity and security of patient health information by adhering to      HIPAA regulations and hospital policies.
    4. Prepare      and organize patient records for coding, billing, and auditing purposes.
    5. Assist      with the release of medical information to authorized individuals and      entities, ensuring proper authorization and documentation.
    6. Conduct      regular audits of health records to identify discrepancies and ensure      compliance with quality standards.
    7. Collaborate      with healthcare professionals, including physicians, nurses, and      administrative staff, to facilitate the accurate documentation and      retrieval of patient information.
    8. Provide      support during the implementation and maintenance of electronic health      record systems and other health information management technologies.
    9. Assist      with the completion of legal and regulatory forms, such as birth      certificates, death certificates, and insurance claims.
    10. Participate      in departmental meetings, training sessions, and continuing education      activities to stay updated on industry trends and best practices.
    11. Compile      and submit credentialing information for insurance credentialing and      practice credentialing.
    12. Prepare      credentialing packets for review by the CMO and Med Staff.

    Non-Essential Functions: 

    • Perform other duties as assigned      or requested.

    Work Environment:

    The HIM Clerk primarily works in an office setting within the hospital. The position may involve occasional exposure to patient care areas and exposure to confidential patient information.

    Physical Demands:

    • Ability to sit,      stand, and move around for extended periods.
    • Manual      dexterity to operate computer keyboards and other office equipment.
    • Vision and      hearing acuity sufficient to perform job duties accurately.

    Qualifications:

    · High school diploma or equivalent required. 

    · Associates degree in health information management or related field preferred.

    · Previous experience in a healthcare setting, particularly in health information management or medical records, is preferred but not required.

    · Familiarity with electronic health record (EHR) systems and other health information technologies is advantageous.

    · Strong attention to detail and organizational skills, with the ability to prioritize tasks effectively in a fast-paced environment.

    · Excellent communication and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams.

    · Knowledge of medical terminology, coding systems (e.g., ICD-10, CPT), and healthcare regulations (e.g., HIPAA) is desirable.

    · Proficiency in basic computer applications, including Microsoft Office suite (Word, Excel, Outlook).

    Certificates, Licensure, Registrations:

    · RHIT or RHIA, preferred.

    Security/Access:

    · Will have access to the facility 24 hours a day.

    · Will have access to confidential information abiding by the organization’s privacy policies and regulations concerning this information.

    · All Harlan County Health System users will be given access to the organizations network to the level required to perform their specific duties. Some users will require access to job specific systems, the authorization of these systems will be granted but the department director or the IT coordinator.
