Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. JOB SUMMARY |
To provide the appropriate level of pharmaceutical care services to patients, while complying with all state and federal laws or rules governing the practice of pharmacy. To facilitate the successful admission and/or discharge of patients to/from home or other facilities, specifically with regard to medications. To supervise any delegated act performed by pharmacy technicians or pharmacy technician trainees.
