Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. A home health nurse is responsible for providing in-home care for elderly and ailing patients who need ongoing attention, but who do not require a hospital stay. The job description of a home health nurse requires patience, compassion, and a specific knowledge of the symptoms and conditions of the patient being cared for.
Position Description
A home health nurse monitors patient’s vitals, distributes medications, ensures patients are comfortable, provides updates to the patient’s primary care physician, and oversees patient rehabilitation and physical therapy.
Essential Duties and Responsibilities
· Provides nursing care and teaching services to patients and their caregivers in their homes.
· Evaluates, coordinates and treats patients utilizing care team model.
· Communicates with DON/Clinical Manager to create the initial plan of care based on patient assessment and physician orders.
· Participate in clinical outcomes monitoring, follow up and agency performance improvement initiatives.
· Oversees coordination of care of care between multiple disciplines in the home.
· Provides updates on patient condition to family and patient physician.
· Assesses and/or manages all in-home medical equipment.
· Acts as liaison between patient, family, and physician.
Education and Experience
