GENERAL DESCRIPTION
The Clinical Pharmacist is a specialized healthcare professional who works as a part of the care team to optimize patient outcomes through the safe and effective use of medications. The Clinical Pharmacist will provide patient consultation, recommendations, and education in accordance with the patients plan of care.

Review records of patients to determine the appropriateness of medication therapy.
Evaluate patients condition to ensure all issues are being addressed and/or treated.
Develop effective medication recommendations that minimize the risk of adverse side effects and ensure appropriate drug therapy.
Review medications for side effects, drug interactions, and proper dosing.
Counsel patients on appropriate medication use and assess patient for barriers to medication adherence in an effort to prevent hospitalizations.
Advise patients on the correct administration of medication.
Recommend alternatives to high-cost medications.
Assess the results of pharmaceutical treatments and recommend alternative treatments if necessary.
Collaborate with healthcare professionals to ensure optimal patient care.
Accurately document medication plans and patient progress in the medical record.
Provide correct documentation to ensure proper billing of visits.
Accurately capture time spent on patient preparation, patient visits, and other tasks related to patient for billing purposes.
Interact with patients via phone, email, patient portal, telephone, text, in person, etc. to gather necessary information.
Provide exceptional customer service to patients, exhibiting a compassionate and empathetic attitude and a patient-centered approach to care. Address patient concerns, inquires, and complaints professionally and promptly, striving to meet patient needs and ensure their satisfaction.
Perform general administrative tasks and support other staff as needed. Utilize the companys software systems to enhance patient care and staff productivity.
Must be available during normal work hours (unless previously approved by direct supervisor). Additional hours may be required to complete normal business functions and/or projects.
Participate in coaching calls.
Perform other duties as requested or required, in the sole discretion of the Company.
