Pharmacist compounds and dispenses prescribed drugs per physician's instructions in a hospital or healthcare facility setting. Consults with medical staff about potential drug interactions, patient medical history, and the use of particular medications. Ensures that all required pharmacy data is recorded and maintained and complies with all regulatory, security and hospital procedures. Additionally, Pharmacist may supervise pharmacy technicians.   • Summary
    The Hospice Clinical Pharmacist position functions as an integral member of the hospice care team, working as the clinical pharmacy specialist in collaboration with hospice care physicians and other health care professionals to ensure that medical therapy aligns with the goals of care throughout the course of treatment.

    RESPONSIBILITIES:
    Recommends goal directed medication therapy to reduce symptom burden
    Answers consults from nurses and caregivers for hospice medication management
    Serves as subject matter expert (SME) for managing hospice medications
    Provides hospice clinical staff with education and training
    Evaluates drug therapy and communicates/documents findings and recommendations
    Recommends medications, including initiation, continuation, discontinuation and altering therapy, based on established protocols
    Assures optimal, effective pharmaceutical care and collaborative practice for hospice patients
    Maintains a current knowledge of all professional and technical developments and requirements affecting the defined area of responsibility
    Prepares information for, and attends IDG (Interdisciplinary Group) meetings when requested
    Processes admission, transfer, and discharge information by adjusting patient data in computer
    Act as an information resource between partner Hospice and Delta Care team
    Assist in EMR order entry upon request
    Performs other related duties as assigned to support achievement of Pharmacy goals and objectives

    REQUIREMENTS:

    RPh required, PharmD preferred
    Full, current and unrestricted license to practice pharmacy in the state of operation (IL)
    Citizen of The United States of America
    Must pass a background/security review
    Experience with various web browsers (Google Chrome / Firefox), Google Docs, Google Sheets or similar software
    Proven ability to manage multiple projects at a time while paying strict attention to detail
    Excellent listening skills
    Excellent verbal and written communication skills (typing)
    Willingness to obtain additional state licensure if necessary

    PREFERRED EXPERIENCE:

    Completion of a PGY-1 or PGY-2 Residency in Palliative Care or Pain Management or equivalent experience in Hospice/Palliative Care/Pain Management.
    Board certification in a relevant specialty certified by the Board of Pharmaceutical Specialties
    Multidisciplinary team leadership experience in a variety of settings
    Experience in collaborative development and launch of new pharmacy or patient care services, including development of policies, procedures, tools and metrics
    Hospice experience
