Pharmacist Manager manages a group of pharmacists who, under the direction of a physician, compound and dispense prescribed drugs in a retail or healthcare facility environment.     JOB SUMMARY:

    Pharmaceutical Care Pharmacist works with physicians, care managers, and other clinical staff as part of a care team focusing on patient outcomes and managing all aspects of patient medication therapy, while maintaining or lowering costs and providing high quality of care. This role assists the physician teams to comply with the accountable care organization measures, working within their scope of practice to manage patient’s medications and working toward improving patient outcomes by providing patient-centered quality of care.

    ESSENTIAL JOB FUNCTIONS:

    Assist physicians in creating and managing the drug regimens of patients with chronic disease states (e.g. diabetes, asthma, CHF etc.), including activities such as meeting with patients, adjusting medication dosages in accordance with PCP, blood sugar and blood pressure monitoring, managing anti-coagulation therapy and performing other services within scope, creating Sync and Thinksync plans, performing medication reconciliation assessments, and coordinating referrals to other clinic services (behavioral health, care coordination, etc.) as needed.

    Meet with patients to provide Comprehensive Medication Reviews (CMR), counsel on the use of various devices (inhalers, glucose monitors and insulin injections etc.), create and follow-up with medication action plans, selecting cost effective, evidence-based regimen, then monitor/adjust prescriptions as needed and monitor patients for appropriate labs and recommended follow-ups.

    Onboard patients to Think Pharmacy, reviewing patient medications and prescription insurance, obtaining information from external sources such as outside providers, Pharmacy Benefit Management, and NeHii as needed to obtain information necessary for onboarding.

    Oversee clinical pharmacy technicians in triaging, reviewing, and authorizing prescription pre-authorizations and refill requests from Think Pharmacy.

    Coordinate and/or participate in population health initiatives to improve medication outcomes for targeted disease states leading to improved health (e.g. Diabetes, COPD, asthma, & CHF).

    Answer patient calls and queries regarding medication prescriptions as need arises.

    Serve as a drug information resource for the providers and support staff.

    KNOWLEDGE, SKILLS & ABILITIES:

    Knowledge of prescriptions, medications, and clinical pharmacy procedures/best practices.

    Knowledge of HIPAA and other privacy rules.

    Skill in using a computer and a variety of software, including Electronics Health Records (EHR) software, Word, Excel, Access, and Outlook.

    Skill in communicating in a professional manner, both verbally and in writing.

    Skill in managing multiple priorities.

    Ability to convey professional demeanor when engaging with patients, protecting privacy, and confidentiality.

    Ability to enter data into a computer to maintain records.

    Ability to follow oral and written instructions.

    Ability to work independently and in a team environment.

    Ability to act as a good representative of the company.

    EDUCATION & EXPERIENCE:

    Pharm D degree from an ACPE accredited institution or Bachelor’s degree in Pharmacy required. Current NE Pharmacy License in good standing required. PGY1 in Pharmacy Practice (Ambulatory Care focus) or PGY1 in Community Pharmacy residency, fellowship, or equivalent clinical experience preferred. Board Certification, prior Electronic Health Record experience and QS/1 System experience preferred.

    WORKING CONDITIONS AND PHYSICAL EFFORT:

    • This role operates in a healthcare setting. This position requires frequent sitting and computer work and allows employee to vary physical position or activity for comfort.

    • Must be able to:

    Stand 15% or longer of an 8-hour workday.

    Walk 40% of an 8-hour workday.

    Sit 45% of an 8-hour workday.

    • Requires ability to lift up to 25 pounds occasionally.

    • Requires employee to: bend, squat, kneel and reach above shoulder level occasionally and twist occasionally.

    • Requires repetitive use of hands for: simple grasping, fine manipulation, computer use

    • Requires all sensory skills (speech, vision, smell, touch, and hearing) corrected to near normal range.
