Physical Therapist - Home Care evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy. Requires a state license to practice physical therapy. Typically reports to a manager. Physical Therapist - Home Care's years of experience requirement may be unspecified.     JOB SUMMARY:

    A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

    QUALIFICATIONS:

    • Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
    • Currently licensed Florida
    • Two years’ experience, preferred.
    • Acceptance of philosophy and goals of this Agency.
    • Ability to exercise initiative and independent judgment.
    • Must have current CPR certification, TB test, Health Statement/physically clear to work, and eligible Level 2 background
    • RESPONSIBILITIES:
    • Understands and adheres to established policies and procedures.
    • Provides physician prescribed physical therapy.
    • Improves or minimizes residual physical disabilities of the patient.
    • Returns the individual to optimum and productive level within the patient’s capabilities.
    • Participates with all other home care personnel in patient care planning.
    • Directs and supervises personnel as required.
    • Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days.
    • Performs all skilled procedures as ordered by a physician.
    • Consults with physicians regarding a change in treatment.
    • Instructs patients and family/significant others in-home programs and activities of daily living.
    • Prepares clinical and progress notes.
    • Assists physician with evaluating level of function.
    • Helps develop the plan of care and revise as necessary.
    • Consults with family and Agency personnel.
    • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
