Physical Therapist - Home Care evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy. Requires a state license to practice physical therapy. Typically reports to a manager. Physical Therapist - Home Care's years of experience requirement may be unspecified.     Job Description:    
       
    A Physical Therapist (PT) will implement treatment programs to assist pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays by planning and administering physical therapy services in the home and community.    
       
    Responsibilities:    
       
    • Provides high quality care and meets the needs of the patient and family by performing evaluations and interpreting assessment results; developing goals appropriate to child; creating and implementing physical therapy treatment plans in conjunction with the physician.    
    • Assists pediatric patients to develop or regain physical, neurological, cognitive and/or social/emotional functioning and improves their level of independence by implementing skilled interventions to maximize the potential of each individual child.    
    • Participates in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine.    
    • Coordinates with referral partners to provide services for children in accordance with the physician order.    
    • Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.    
    • Documents patient care services and care coordination in an intuitive electronic medical record system.    
    • Maintains patient confidence by keeping information confidential.
