Physical Therapist - Home Care evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy. Requires a state license to practice physical therapy. Typically reports to a manager. Physical Therapist - Home Care's years of experience requirement may be unspecified. Job Summary
- The Physical Therapist Assistant is accountable for being an active member of the Avera Therapy rehabilitation team, to provide care and treatment of assigned patients/clients/students/residents in a caring, safe and efficient manner.
- Provides exceptional care and service to patients, residents, guests and fellow employees at all times.
- Performs all duties and responsibilities in such a way as to demonstrate and support the hospital's Catholic heritage and Avera's values of hospitality, compassion and stewardship.
- Provides care/service in accordance with regulatory and accrediting agencies standards and Catholic Ethical and Religious Directives.
Education and/or Experience
- Graduate of an accredited Physical Therapy Assistant Program required.
Licensure/Certification/Registration
- Licensed as a Physical Therapy Assistant in South Dakota required.
- BLS Certification required.
Work Schedule
- PRN - Varied Shifts, as needed, casual requirements per department policy
Pay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
