    Employee Status: Exempt

    Position Summary: Under general supervision, but in line with established PC&HC policies and procedures, provides patient care; conducts physical exams; completes health history summary; documents patient progress; assists with patient care management.

    Essential Functions

    • Represent PC&HC to patients/visitors in a caring, courteous, and professional manner.
    • Provide prompt, efficient and accurate patient care.
    • Deliver and direct patient care within the scope of practice.
    • Interpret laboratory and diagnostic findings, taking action as needed.
    • Complete documentation on all patients using E-Clinical software within 48 hours of patient encounter.
    • Review patient health records.
    • Address patient prescription requests and other needs per the Electronic Medical Records within 24-48 hours of request.
    • Consult and coordinate with health care providers and case managers by answering questions regarding patient care and developing treatment plans for patients.
    • Refer patient to medical specialist or other practitioner when necessary.
    • Contact drug companies as directed for medication samples.
    • Complete all required paperwork.
    • Manage patient care after hours via on call rotation and participate in weekend shift rotation as assigned by health center management.
    • with the Medical Director and CEO in the development/update of practice protocols, policies and procedures.
    • Provide medical perspective and leadership in critical incident reviews and other medical practice aspects of risk management.
    • Support, communicate and ensure compliance with policies, procedures, bylaws, regulatory requirements, and best practice guidelines.
    • Participate in the quality assurance program including monitoring quality of care delivered, reviewing charts and providing/receiving consistent feedback on the quality of care being delivered.
    • Ensure a safe work environment for staff and patients, including, following infection control and OSHA standards and assist in disaster planning/activities.
    • Practice Patient Centered Medical Home concept and maintain quality and meaningful use standards.
    • Treat all clinic and employee information confidentially and in accordance with established guidelines.
    • Plan and organize work load to maintain efficient patient operations in the Health Center.
    • Maintain a strong patient focus while working collaboratively within the teams multiple demands and patient needs.

    • Must be able to lift 30 pounds/ walk and/or stand for 8 hour shifts-bend or stoop throughout shift.
    • Must have ability to work as a team player.
    • Must have computer skills with ability to multi task.
    • Must communicate with patients/visitors and staff in a friendly, positive, and professional manner.
    • Must follow all HIPAA and OSHA requirements.
    • Must be supportive and participate in our mission in order to provide healthcare to the underinsured and uninsured.

    Education/Certification Requirements:

    • Must have a Doctoral Degree in Family Medicine and be Board Certified.
    • Must have completed a residency program.
    • Must maintain a CPR Certification.
    • Must maintain all licensure and certifications to perform duties as required.
    • Must complete continuing medical education to meet organizational and state licensing requirements.
