    As an accredited public health department, we're seeking highly qualified candidates for the position of Communicable Disease Specialist 3 to join the North Central Health District 5-2 in Macon, GA.

    Job Responsibilities

    Assists in the implementation of timely disease intervention strategies to decrease the prevalence of communicable diseases.

    Performs investigations, data collection and analysis, and interpretation.

    Performs reporting and surveillance activities.

    Prepares documentation of communicable disease case investigations occurring within the district.

    Provides case management services, eg home visits, clinic referrals, patient education/counseling, etc to clients with various communicable diseases such as sexually transmitted diseases, tuberculosis, HIV/AIDS, hepatitis, etc.

    Provides direct diagnostic and therapeutic services for public health clients both in the clinic and field setting.

    Provides educational programs, seminars and in-service training sessions to enhance public understanding of various communicable diseases.

    Ensures implementation of timely disease intervention strategies to decrease the prevalence of such diseases. Works with the HIV Linkage Coordinator to link HIV-positive individuals to care and re-engages HIV-positive individuals back into care. 

    Collaborates with Communicable Disease, Linkage Coordinator and other NCHD staff on surveillance and quality management to identify patients who are lost-to-care and are unable to be brought back into care by clinic staff.

    Investigates and locates patients who are lost-to-care; determine if and where they are engaged in care.

    Plans and coordinates delivery of intervention activities. Maintains records, collect data, SENDSS data, entry develops and implements care plans. 

    Maintains consistent communication and coordinates services and activities with Communicable Disease Supervisor, Linkage Coordinator and program staff. Assists with the preparation and submission of monthly service reports and quarterly narratives. 

    Meets regularly with a multi-disciplinary team. Attends conferences, educational webinars and meetings to increase knowledge of best practices. 

    Utilize the Program Collaboration and Service Integration with all staff of the Office of Epidemiology, Community Assessment, and Research initiatives.

    Minimum Qualifications

    Bachelor's degree from an accredited college or university AND One year of related experience

    OR

    Associate's degree from an accredited college AND Three years of experience in a related area

    OR

    Five years of experience in a medical related field

    OR

    licensure as a Registered Nurse AND Two years of nursing experience

    OR

    One year of experience at the lower level (HCP081) or position equivalent.

    Bachelor's degree from an accredited college or university AND One year of related experience OR Associate's degree from an accredited college or university AND Three years of experience in a related area OR Five years of experience in a medical related field OR licensure as a Registered Nurse AND Two years of nursing experience OR One year of experience at the lower level Communicable Disease Spec 2 (HCP081) or position equivalent.
