    Job Summary:

    The Family Practice Physician will provide compassionate care at a facility in Ft. Duchesne, Utah

    Tour of Duty: Monday-Friday 8:00am-5:00pm.

    Supervisory Responsibilities: None


    Duties/Responsibilities:

    • Assesses and perform outpatient and inpatient (if desired) family practice duties and manage patient's needs.
    • May perform minor office surgery procedures.
    • Inpatient includes Medical Surgical Ward, Pediatric Ward, Intensive Care Unit, Labor & Delivery/Post-partum Ward.
    • Provide consultation with members of the Family Medicine staff on matters of patient care.
    • Review clinical records and adhere to records documentation procedures to ensure records are properly maintained in the appropriate system.
    • Other duties as assigned based on experience and education.

    Education and Experience:

    • Required: Current Board certification by the American Board of Family Medicine, Internal Medicine, or Pediatrics with the exception being that they are Board eligible
    • Minimum Education: Doctorate degree of Osteopathic or Allopathic Medicine.
    • Required: Shall have a minimum of thirty-six (36) months of experience within their specialty.
    • Required: Possess a current, valid, unrestricted medical license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract. License verification must be provided at the time of application.
    • Required: Maintain valid and current certification and provide proof at the time of application in the following:
      • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS).
    • Required: Possess and maintain a current ID or driver's license in any state.
    • Required: Provide three (3) professional references at the time of application. One letter shall be from the Chief of Staff or Lead at the hospital where the physician holds or last held staff privileges.
    • Required: Does not have, nor has ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or denied upon application.
    • Required: Technically proficient in the skills necessary to fulfill the government’s requirements, including the ability to speak, understand, read, and write English fluently.
    • Required: Must be able to pass a federal background check.
    • Medical license verification, CV/resume, references, and certifications must be provided at the time of application and can be emailed to certification@chloeta.com.

    Physical Requirements:

    • Must be able to withstand physical and emotional stress.
    • Must be able to stand and move for long periods of time.
    • Able to frequently lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.
    • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
    • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

    Pre-employment Requirements:

    Drug and Alcohol Testing:

    To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.


    Fair Credit Reporting Act (FCRA):

    Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
