    Bluestone Physician Services is seeking a part-time physician to be a consultative partner and provide clinical collaboration for our advanced practice providers who provide on-site care to geriatric patients who live in Assisted Living and Memory Care Communities in and around Racine, WI. We are a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive and includes all members of the care team, patients and their families. Our primary care providers practice within a team-based model of care comprised of care coordinators, nursing and behavioral health professionals. 

    Our Care Model delivers customized care that is preventive, practice, and includes all members of the care team, patients and their families. Our patients experience 21% less ER visits, 36% less hospitalization and 41% less hospital re-admissions compared to patients with similar conditions and complexities. 

    Role Overview:

    • Provide phone consultation and collaboration with 5-6 advanced practice providers (approximately 3-4 calls per/day)
    • Provide in person consultative care approx. 2 days per week within 1-2 buildings per day. 
    • Consultative Physicians do not manage their own panel of patients
    • Able to respond to consultative request within 2 hours 
    • Easy technology to effectively communicate between the Physician and Nurse Practitioners and Physician Assistants
    • Participate in taking “call” from time to time (recommended)
    • Receive an annual stipend for oversight
    • Ideal for physicians who want to ease out of the day-to-day clinical practice but who still want to connected to patient care

    Qualifications:

    • Graduate of an accredited ACGME/AOA residency training program
    • Board Certified or board prepared with the American Board of Medical Specialties or American Osteopathic Association 
    • Licensed or ability to practice within the State of Florida
    • Ability to maintain DEA certification for the State of Florida
    • Proven ability to communicate effectively with strong verbal skills
    • Excellent interpersonal and customer service skills
