Plan Design Specialist works closely with clients to design group health, dental and/or life insurance plans to create an overall benefit package. Works alongside underwriters, but is not an underwriter role. Being a Plan Design Specialist monitors and analyzes cost trends. May handle more complex plan design. Additionally, Plan Design Specialist requires a bachelor's degree or its equivalent. Typically reports to a supervisor or manager. The Plan Design Specialist gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Day to Day:
• Identification and outreach to individuals and families to identify those in need of health coverage and / or a health care home, including efforts targeting special population (migrants and homeless).
• Provide application assistance for health coverage and follow up as needed to assure affordable health coverage is obtained whenever possible.
• Provide assistance in accessing entitlements and completing forms and applications.
• Provide assistance to assure coverage is maintained.
• Provide education to individuals and families regarding effective use of the local health care system.
• Document activity accurately and efficiently and as requested by programs.
• Provide holistic information about available services to address client needs - at YNHS and other agencies.
• Provide assistance with securing food, shelter, clothing and referrals to other supportive services.
• Develop individual care plans in consensus with staff and client.
• Crisis intervention.
• Advocate on behalf of clients.
• Maintaining confidentiality of client interactions and medical records.
• Participate in staff meetings.
• Participate in quality assurance activities and peer review as requested.
• Maintain organized, clean, efficient and confidential work area.
• Represent YNHS in local meetings as requested.
• Perform related work and other tasks as requested.
