Reporting to the Chief Financial Officer, the Case Management Director will collaborate with hospital leadership and other Nursing Directors to effectively manage hospital wide care coordination. The Case Management Director will oversee a team of Registered Nurse Case Managers and Licensed Social Workers.
The Case Management Director should have over 5 years of experience working as a Case Manager in an inpatient setting as well as progressive leadership experience. The Director will oversee both the Case Management Department and the Social Services Department. The Case Management Director will be post overseeing the organization of complex care, assessment of patient's health insurance plans and provision of necessary resources for patients being discharged. The Director of Case Management will act as an advocate for their patients and go above and beyond to help accurately assess their individual needs.
