The Center for Human Development (CHD) is currently seeking an experienced and creative Case Manager at its Two Rivers Recovery Center in Greenfield. Two Rivers is a program for women in early recovery from substance use who are pregnant or postpartum mothers with their infants. The program supports up to 25 women and 5 infants in long term (6 to 12 months) residential treatment. The comprehensive clinical service is a trauma informed, harm-reduction model.
The Case Managers is responsible for the development and implementation of person-centered treatment plans, facilitating evidenced-based therapeutic group, individual therapy, assistance with discharge planning, as well as community support coordination and collaboration. The Case Manager receives supervision from a professional, experienced Clinical Director.
Qualifications:
Bachelors Degree in a related field is required; experience in clinical positions; knowledge of substance use disorders and treatment; experience facilitating groups; one weekend day required.
