  • We are seeking a dedicated and compassionate Part-Time (20 hr per week) Social Worker to join our skilled nursing facility in Sudbury, MA.

    At Bear Mountain Sudbury, we provide post-acute services, rehabilitative services, skilled nursing, short- and long-term care through Social Services, Physical, Occupational, and Speech Therapists; Registered and Licensed Practical Nurses; and Certified Nursing Assistants. Our experienced staff of professionals are here to meet the needs of every resident.

    The Social Worker will play a crucial role in providing comprehensive psychosocial support to our residents, promoting their mental and emotional well-being. The ideal candidate will provide an environment that upholds residents' individuality, independence, and dignity while striving to enhance their overall quality of life.

    Responsibilities:

    Psychosocial Assessment and Care Planning:

    • Conduct thorough social histories and psychosocial assessments for each resident, identifying their social, emotional, and psychological needs.
    • Collaborate with the interdisciplinary team to develop personalized care plans that address residents' psychosocial needs and set achievable goals.
    • Provide therapeutic interventions to assist residents in coping with the transition to long-term care, supporting their social, emotional, and psychological well-being.

    Advocacy and Resident Rights:

    • Empower residents, families, and staff to take an active role in self-advocacy, enhancing the quality of life and care within the nursing home and the larger community.
    • Ensure compliance with laws and professional licensure requirements related to resident abuse prevention and reporting.

    Community Resource Management:

    • Develop, maintain, and utilize a comprehensive listing of current community resources to support residents and their families/significant others.
    • Stay updated on available social service agencies and refer residents and families when needed services are not provided by the facility.
    • Collaborate with the interdisciplinary team to review and establish social work policies to optimize resident care and quality of life.

    Documentation and Quality Assurance:

    • Review facility policies and procedures to ensure compliance with state and federal regulations as part of the interdisciplinary team.
    • Document progress in meeting residents' psychosocial needs and maintain accurate social service documentation.
    • Actively participate in interdisciplinary team meetings for quality assurance purposes.

    Discharge Planning and Coordination:

    • Coordinate the resident discharge planning process, ensuring a smooth transition back to the community.
    • Make appropriate referrals for home care services and facilitate residents' access to necessary resources.

    Qualifications:

    • Meet academic credentials mandated by the current state licensure requirements.
    • Licensure: Possess a valid LSW, LCSW, or LICSW license (MSW is a plus)
    • Experience: Prefer prior experience working with elders in a community or long-term care setting.
    • Excellent communication, assessment, and social work methods and techniques. Ability to work effectively with diverse disciplines in both individual and team settings. Working understanding of social systems and the ability to implement relevant state and federal regulations.
