We are searching for a full time Rehabilitation Case Manager (RCM) to work and support our Maryland programs. This position is responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up. This role coordinates schedules, therapies and other appointments/services, and is responsible for regular contact with families and funders, as well as ongoing internal report writing, clinical documentation and facilitating funding issues as needed. The individual serving in this position acts as a client advocate and is responsible for client treatment reports and chart documentation.

A bachelor’s degree along with case management and health care industry experience is required. A master’s degree and CRC or CCM designation is preferred. A valid driver's license, insurance and responsible driving behaviors must be maintained. Excellent planning, organizational, writing and time management skills are required.
