    Job Summary:

    The Bachelor of Social Work (BSW) is responsible for providing and coordinating behavioral health services for patients as part of an integrative model of care within the primary care setting. The BSW collaborates with and assists primary care and behavioral health staff as well as other internal and external service providers in order to effectively manage mental health and substance use conditions and support physical health outcomes. The BSW coordinates behavioral health referrals, screens patients using structured instruments, educates patients about mental health and substance use conditions and treatment options, and monitors symptoms and response to services.

    Under the direct supervision of the Director of Behavioral Health, this position supports the TCC clinical care team, including LCSWs, LMFTs, AMFTs, ACSWs, Clinic Managers and Primary Care Providers and other staff. Independent judgment is required to plan, prioritize and organize diversified caseload with professionalism and discretion. The position continually requires demonstrated poise, tact and diplomacy in working with a diverse group of internal and external partners.

    This position specifically supports the development and provision of trauma-informed care services across the lifespan.

    Essential Job Functions:

    • Respond to “warm hand offs” and referrals from primary care providers.
    • Screen patient needs to determine the appropriate level of services, in consultation with supervision.
    • Based on patient needs, provide case management services to coordinate needed mental health, physical health and substance use resources and supportive services.
    • Make referrals for internal therapy for mild to moderate patients or external referrals for specialty mental health services.
    • Follow-up on referrals of patients through in-person visits, phone calls and letters.
    • Assist the care team with assessing patients, developing treatment plans and interventions.
    • Actively engage patients in obtaining needed behavioral health and physical health services.
    • Monitor patient progress with goals and provide appropriate follow-up services.
    • Schedule routine appointments and manage schedules so as to address emergent issues in a timely manner.
    • Provide linkage to services and coordinate communications between internal and external service providers.

    • Obtain appropriate consultation with supervision as needed.
    • Complete written documentation according to program standards and timelines in the electronic health system.
    • Participate in individual and group supervision as required.

    Other Duties:

    • Answers telephone, screens callers, relays messages, and greets visitors in a professional and compassionate manner.
    • Complete and prepare various types of administrative medical forms for outside agencies.
    • Other duties as assigned.

    Knowledge, Skills and Abilities:

    • Bachelor of Social Work (BSW) degree from an accredited University.
    • Knowledge of a range of community resources related to mental health and supportive services.
    • Proficiency in a variety of computer software applications such as word processing, spreadsheets, database and presentation software (i.e. Microsoft Word, Excel, PowerPoint, Outlook, Adobe Acrobat), including a working knowledge of HTML.
    • Excellent written and verbal communication skills.
    • Skills in multi-tasking in a high stress environment while working with a variety of clients.
    • High level of interpersonal skills to handle sensitive and/or confidential situations.
    • Analytical ability is required in order to gather and summarize data for reports, find solutions to various administrative problems and prioritize work.
    • Continual attention to detail in composing, typing and proofing materials, establishing priorities and meeting deadlines.
    • Electronic Medical Record (EMR) proficiency.
    • Bilingual English/Spanish preferred
    • Medical and insurance terminology is a plus.
    • Ability to communicate and comprehend in written and verbal form using correct grammar in English.
    • Ability to use mathematical and analytical skills.
    • Ability to treat individuals of diverse ethnic and cultural backgrounds with respect and exceptional customer service.
    • Ability to work independently, show initiative and work productively within a team environment.
    • Ability to maintain a high level of professionalism and confidentiality.

    Physical Demands:

    • While performing the duties of this job, the employee is required to frequently sit in upright position at a desk for office work.
    • Employee may be required to stand for long periods of time and move rapidly in a fast past office environment. Employee may occasional be required to drive to attend outside meetings and trainings.
    • Employee may be required to climb stairs to get to restroom facilities and employee lounge.
    • Employee will be required to use telephone system and office equipment regularly.
    • Employee will be required to use repetitive hand and finger motion to type using a variety of computer applications including practice management system, internet and other office equipment.
    • The employee is required to sit, talk and pay close attention to detail in a busy office environment.
    • The employee will occasionally be required to lift and/or move up to 10 pounds.

    Work Conditions:

    Work Hours:

    • Eight hour work days with variable/flexible hours depending on patient care needs. Overtime may be required.
