As a Case Manager Social Worker, you’ll fill many roles as you facilitate seamless care for every patient you serve. You work closely with patients, family members, social workers and caregivers to:
• Assess insurance plans.
• Assist with transition care management.
• Provide education and support to patients based on diagnosis.
• Provide resources available for patients to help in their recovery.
The ideal caregiver is someone who:
• Currently has a state licensure as a Social Worker and Basic Life Support (BLS) certificate.
• Shows strong critical thinking skills and decisive judgement under minimal supervision.
• Works well in a close-knit teambuilding environment.
• Can be open to new ideas and changes in the work department.
