Professor - Social Work jobs in Nassau, NY

This position requires the performance of limited social work in helping residents and members of their family with social, emotional and related difficulties associated with their medical condition. The work is performed under the supervision of a qualified social worker who is available either on a consulting or direct supervisory basis. Independence of action will vary depending upon whether or not the incumbent is under direct daily supervision. Employees in this class are expected to confer with professionals or agencies in or outside of the facility to obtain assistance and guidance for residents. Performs other related work as required.

Responsibilities/Typical Work Activities:

• Provides social work services to help residents and members of their family adjust to their illnesses or disabilities;
• Collects and maintains appropriate data from various sources such as staff and residents’ families to establish an overall approach to solution of medical and social problems;
• Maintains information on community and health resources which can be utilized during the residents’ care and subsequent to his discharge;
• Records social information to develop residents case histories;
• Participates in the process of transferring residents to other levels of medical care as appropriate.
• Complete social work assessment of residents upon admission and readmission;
• Develop and provide social work recommendations to formulate resident care plans;
• Liaison to residents’ families and social service agencies with supervision from director as needed;
• Attend care conferences as required by MDS guidelines;
• Advocate resident/family concerns in a timely manner;
• Facilitate, within reason, resident and family personal requests to maximize quality of life;
• Educate residents, family and staff about resident rights, advance directives per state and facility guidelines;
• May participate in quality assurance monitoring and audits and serve on QA subcommittees.

• Working knowledge of the practices and procedures of social case work and of the social factors related to disease and disability;
• Ability to organize work effectively;
• Ability to prepare reports;

• Physical condition commensurate with the demands of the position.

Minimum Qualifications:

Graduation from a regionally accredited or New York State registered four-year college or university with at least a bachelor's degree in social work or related field.
