The role of the Special Populations Patient Navigator is to function as part of a Care Team and to provide a wide range of administrative, patient support, and care coordination functions for patients considered Special Populations to include, but not limited to, behavioral health, Ryan White (RW), Healthcare for the Homeless (HCH), persons living with Hepatitis C, participants in the American Cancer Society (ACS) screening program, and Children & Youth with Special Healthcare Needs (CYSHCN) program participants.

To qualify for this position, a high school diploma or General Equivalency Degree (GED) is required. Minimum Experience: 3 years of experience in a health care setting preferred. Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying. Required Licenses/Certifications: Valid state-issued ID.
